Responsibilities

• Provide direct care and support to individuals with diverse abilities in their home and community settings.
• Assist individuals with daily living activities, including personal hygiene, meal preparation, and household chores.
• Foster a supportive and empowering environment that encourages individuals to actively participate in decision-making and goal setting.
• Implement individualized care plans and support strategies, ensuring the individual's unique needs and preferences are met.
• Promote community integration and socialization by assisting individuals in participating in recreational activities, community events, and social outings.
• Monitor and document individual progress, behaviors, and any changes in health or well-being, reporting observations to the appropriate supervisor.
• Collaborate with a multidisciplinary team, including families, caregivers, and healthcare professionals, to provide comprehensive care and support.
• Adhere to all safety protocols and procedures, ensuring the physical and emotional well-being of individuals at all times.
• Maintain confidentiality and respect the privacy rights of individuals and their families.

Qualifications

• High school diploma or equivalent (additional education or training in a related field is a plus)
• Previous experience working with individuals with diverse abilities or in a similar direct support role is preferred.
• Strong interpersonal and communication skills, with the ability to build rapport and establish positive relationships with individuals and their families.
• Empathy, patience, and a genuine desire to make a positive difference in the lives of others.
• Ability to work independently as well as part of a team, demonstrating flexibility and adaptability in a dynamic environment.
• Basic knowledge of healthcare and safety practices.
• Valid driver's license and reliable transportation (if required for community-based support).
• Ability to pass a background check and meet all state and local regulatory requirements.
